Notes on creating pdf forms...

I was instructed to convert a technical survey document to a paperless pdf format. This is the first time I'm using Adobe Acrobat Professional 9 to create a pdf form, so I'm posting all the links and references for this process.

Once you created a pdf form, you can even add a submit form feature. This enables people to send the form to a specified email address once they finished filling up the form.
